The following information will be made available to researchers upon submission of a data use agreement from a secure website maintained by the ATHLOS Consortium: 1) the information extracted and documented from each survey; 2) survey files with the original variables as well as transformed variables; 3) a compiled data set and the algorithms used to generate common variables with complete metadata that meet Data Documentation Initiative (DDI) standards; and 4) all syntax or algorithms used to perform the data analyses, with complete documentation of rationale, choice of models and procedures.

A stratified two-stage cluster sampling design was used, and strata were created based on the largest towns and university hospital regions. A systematic sampling of people was conducted so that the sample size in each stratum was proportional to the corresponding population base.

A stratified multistage random sampling method was used and strata were created according to the geographical administrative regions and number of people living in the habitat. Age strata were used to select households according to the age structure of the population. The respondents were randomly selected among inhabitants of a household from a certain age group.
